Memory & Dementia Care at Home
Our care ensures peace of mind for you and your loved one. Our trained team delivers compensatory strategies, personal care, predictable routines to reduce agitation with patience, encouragement ant empathy. We make necessary adjustments as progression happens, provide cognition training and stimulation with meaningful and purposeful activities. We maintain GPS monitoring with a tracker watch.
Home Again Transitional Care
Our care ensures peace of mind for both the individual and their loved ones after a hospital or nursing home stay. Our team will work with you to develop compensatory strategies, provide personal care, make the return home seamless, prep the home for return – clean linens, stock fresh groceries, do light housekeeping. We also pick up prescriptions, provide medication reminders and transportation to follow-up appointments and can help with exercise programs.
End of Life Care
Our Personal Caregivers take time to understand personal preferences and habits so you can be a family member to your loved one instead of a caregiver at end of life. Our trained team partners with hospice agencies as needed. Services include hygiene, delicate skin care, feeding, dressing, toileting, transferring, repositioning, hand holding and light housekeeping.
Bed-bound Care
We are experts in assisting loved ones who must remain in bed. We are able to assist with turning, providing baths in bed and skin care.
Adaptive Equipment
We are specially trained to assist our clients with their adaptive equipment such as a Hoyer Lift or sit-to-stand lift, allowing loved ones to be transferred from one place to another.
