ServicesSenior Care
Personal Caregivers assist with daily activities of living making it possible for your loved one to age better at home. Our care services are available hourly and also up to 24/7.
Senior Care covers a broad range of services …
Our caregivers assist with the routines of daily living: grooming, dressing, bathing, hygiene, toileting, mobility, transferring and monitor for changes you may need to be aware of.
We can provide cooking services and companionship – taking your loved one to visit friends, shopping, on social outings. Our caregivers give support through quiet presence or active conversation and activities. They are expert at understanding different needs at different times.
Our Companion Care services can include: shopping and errands, social outings, visitation, hobbies, games and projects, light housekeeping and laundry, meal planning and/or preparation and medication reminders. We work with you to customize the services we’ll provide to bring comfort and ease for everyone.
Home Safety Evaluations
A member of our team will conduct a home safety evaluation prior to the start of care to offer recommendations about how to make your home safe for your loved one.
Here are six easy suggestions:
Make sure that hallways and staircases are well-lit.
Remove tripping hazards from walkways.
Lay down non-skid mats under rugs.
Store commonly used items in easy-to-reach cabinets.
Provide a dining chair with side arms.
Make sure wires and cords are tucked out of the way.
Home Safety Equipment
Home Care Concierge is dedicated to helping our clients age safer at home. For home safety equipment needs, we refer clients to Connect America, the nation’s largest independent provider of personal emergency response systems. Their solutions include:
Fall Detection
The Fall Detection Pendant is a safeguard designed for situations when an individual falls and is unable to push a help button. When a fall is detected and no sudden movement follows, Connect America is alerted. Then, Connect America will communicate with the fallen person to ensure their safety. In the event of an emergency, a caregiver, neighbor, or emergency personnel will be contacted.
Medication Management
Connect America’s medication monitoring unit, MedReady, organizes and dispenses medications according to the physician-prescribed schedule. When not in use, the medication monitoring unit is automatically locked and secured to prevent tampering or potential overdose.
Remote Patient Monitoring
Clients can better manage chronic illnesses including Diabetes, Congestive Heart Failure, Hypertension, COPD, and more through Connect America’s vital signs monitoring system. This system allows patients in need of intervention to be connected directly for help.
