How Hospice and Home Care Work Together

When a loved one is nearing the end of life at home, it’s important to have a caring hospice team to give them the care they need and to support your family. Unfortunately, regular visits from home health hospice nurses aren’t always enough. That’s where home care can be an invaluable support to your loved one and your family.

Our caregivers can spend much more time in the home attending to your loved one’s physical and emotional needs. They provide personal care on a daily basis, paying close attention to how your loved one’s condition is progressing. Our Home Care Concierge team members can then coordinate with your loved one’s physicians and hospice agency ensuring that they are kept as comfortable as possible until the very end.

Our caregivers also help your loved one and their families deal with the complex emotions that arise during hospice care. They can help you emotionally prepare for the loss of life and understand what to expect as your loved one’s disease progresses. They may also help with practical matters such as light housekeeping, hygiene and personal care, mobility care, transportation and meal preparation.
